Before Beginning an Online
Application
Get organized!
To simplify the application process, gather required documents and other information
ahead of time.
Collect Required Documents. Following are the required documents
that you should mail to us after you complete the Online Application.
A photocopy of each item is sufficient for our record.
Should the original is required, we will ask for it at later time.
As many pieces of information are required from these documents, during the
Online Application, we recommend you collect the required documents before you begin
the application process.
Applicants in Foreign Countries.
The majority of the required documents are for applicants who are currently staying
the
U.S. You may skip the items that do not
apply to you or your eligibility for the
U.S.
permanent residency.
For Principal Applicant:

Passport Biographical Information Page.

Original Visa used for last entry into the
U.S.
-
I-94 Departure Record.
-
I-797 USCIS Approval Notice for your visa petition, if applicable.
-
I-797 USCIS Approval Notice for any Change of Status or Extension of Status after
arrival in the
U.S.
, if applicable.
-
All DS-230 Certificates of Eligibility, if a J-1/J-2 visitor.
-
All I-20 Certificates of Eligibility, if a F-1/F-2 student or family.
-
Any other immigration papers issued by U.S. Government for your current visa status.
-
Professional Degrees (Doctor’s or Master’s) and
the Transcripts, if applicable.

Bachelor’s degree and the Transcript.
-
Credentials Evaluation for all degrees
and transcript received from foreign institutions.

State Teaching Certificate, if applicable.
-
Resume.
-
Reference Letters for your work experiences only related to the position
offered by your sponsoring employer for the permanent residence application.
For Each of Family Members:

Collect Necessary Information for
Online Application. Following
are additional pieces of information you need during the application process. If necessary, please arrange referential
documents in advance, for your convenience in the application process.

Marriage: Official date and place
-
Information about previous residences (last 5 years): address, month and year you
moved in and out the residence.

Information about previous employment (last 5 years): employer company name and
address, month and year you started and left the job
-
If a H-1B/H-4 worker and family, information about your previous stays in H status:
dates for all
U.S.
arrivals and departures.

Parents: dates and places of birth, if known.
-
Former spouse, if applicable: dates and places for the marriage and termination.
Remember to Sign and Send Certification. At the end of online application, you
are required to certify the correctness of the data you entered.
Your certification is executed by clicking the <submit> button placed
at the last page (“Certification”).
The application system will pop up a hard copy of your Certification, along with
the final instruction. Please make
sure you follow the end instruction before you leave the application process.
Tips for Online Application.

Should you stop in the middle of the online application, you may do so and return
to the application system at later time.
Please make sure to save your data entries before you leave.
The <Save> button is placed at the bottom of each page.

Questions are grouped by Sections, and each group is shown on one page.
When you move to next Section (page), the application system automatically
saves your entries. If you should stop,
however, in the middle of the section, please click the <Save> button before
you leave.

Sample answers are provided for your reference, in case you are unsure of questions.

For technical immigration data required, we provided illustrations for your easy
reference. Click the question mark
buttons for those items.
- If necessary, you
may jump to other Sections or other Family Member Data pages, by clicking the connecting
buttons or links placed at the top of each page.
- Summary pages contained a <print> command button
for your convenience. In case you wish
to print out other pages, please use the regular print command provided by your
Internet browser.
